Please complete all areas on the questionnaire page as this will assist with the creation and accuracy of your poem. Once the information is received and processed, a poem will be drafted and you will be e-mailed, faxed, or mailed a copy of the first draft for your review.

After your review, please contact Poetic Moments by phone, e-mail, or mail with your name, order number, and any necessary changes. Once we have implemented any suggestions or changes, your poem will be printed in final form and mailed out within three (3) business days

We strive to create the best quality, and if you are not completely satisfied, please notify Poetic Moments within three (3) days of receiving your finished product.
